29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from fallingor flying objects, or from electrical shock and burns, were not protected by protective helmets. A. J.E. Stang Masonry, Ltd. worksite located at 3298 Sterlingwood Lane, Perrysburg, Ohio: On or about February 10, 2012, the employer did not ensure employees were protected from head injuries while working below employees performing masonry activities. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portableladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps,walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Crossbraces were used as a means of access. A. J.E. Stang Masonry, Ltd. worksite located at 3298 Sterlingwood Lane, Perrysburg, Ohio: On or about February 10, 2012, the employer did not ensure a means of access was provided for an employee accessing the tubular welded frame scaffold system which was four frames high. The exposed employee was climbing cross braces to access the platform.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of thissection, each employee were not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4)of this section. A. J.E. Stang Masonry, Ltd. worksite located at 3298 Sterlingwood Lane, Perrysburg, Ohio: On or about February 10, 2012, the employer failed to ensure employees were protected from fall hazards while performing bricklaying work from a tubular welded frame scaffold system four frames high. Employees were exposed to a fall distance of approximately 20 feet.
