29 CFR 1910.1030(f)(2)(i): The employer did not ensure that hepatitis B vaccination was made available after the employee had received training and within 10 working days of initial assignment to all employees with occupational exposure:    (a) On or about February 7, 2012 and at times prior, the employer did not ensure that the employees who accepted hepatitis B vaccination received their shots in a timely manner. The employer had several employees, still currently working on site, who did not receive the vaccinations as requested.

29 CFR 1910.1030(f)(2)(iv): The employer did not ensure that employees who declined to accept the hepatitis B vaccination offered by the employer signed the statement in appendix A:    a) On or about February 7, 2012 and at times prior, the employer did not ensure that the employees who declined the hepatitis B vaccination signed the declination statement. The medical records for several current employees who declined the vaccinations, did not contained signed declination forms.

29 CFR 1910.1030(f)(5): The employer did not obtain or provide the employee with a copy of the evaluating health care professional's written opinion within 15 days of the completion of the evaluation:    a) On or about February 7, 2012 and at times prior, after a needle stick incident on June 25, 2011, the employer failed to provide a copy of the evaluating healthcare professional's written opinion to the employee.    b) On or about February 7, 2012 and at times prior, after an exposure incident on September 11, 2011, the employer failed to provide a copy of the evaluating healthcare professional's written opinion to the employee.
