29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.    Location: On scaffold, inside of building;    On or about 02/09/12;    a)  Employees were working on a scaffold that did not bear mudsills or baseplates and was erected on various pieces of plywood.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.    Location: Scaffold inside building;    On or about 02/09/12;    a) Employees working on a tubular welded scaffold were exposed to a fall of approximately 40 ft.  The employees were not using personal fall protection nor were guardrails installed in the areas they were working.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:    Location: On scaffold; inside building.    On or about 02/09/12;    a) Employees were working on a tubular welded scaffold without the use of a fall protection system and were not able to recognize fall hazards.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: Roof of building;    On or about 02/09/12;    a) Employee was exposed to a fall of approx 40 ft while working on the edge of roof with no fall protection.
