Temporary wiring was used in lieu of fixed wiring.  2) Lower body shop, an extension cord was used to service the permanently located automatic release for the air filtration system.  Temporary wiring was used in lieu of fixed wiring.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    Visual inspections were not conducted as evidenced by:    1)  Lower body shop, the SP-125 Plus Lincoln Electric MIG welder (110V, 125 amp) had a missing ground pin.   2)  Lower body shop, there was lack of strain relief on the extension cord and plug servicing the CAR-O-LINER power lift.  3)  Storage closet,  the SP-135T Lincoln Electric MIG welder had damaged outer insulation.
