29 CFR 1926.652(a) (1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a) Naranjo Civil Constructors @ Harmony Ave., & Zuni Street, Broomfield, CO. On or before February 1, 2012, the employer did not ensure that each employee conducting work in an excavation was protected from a possible wall cave-in, in that the excavation was approximately 15 feet deep lacked an adequate protective system, such as appropriate sloping, benching, or shield system on all unprotected sides. This condition exposed the employees to a crushing hazard by excavation wall cave-in.

29 CFR 1926.21(b) (2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: (a) Naranjo Civil Constructors @ Harmony Ave., & Zuni Street, Broomfield, CO. On or before February 1, 2012, the employer did not ensure that each employee conducting work in an excavation was provided with adequate training regarding the hazards associated with trenching and excavation as required under the standard. The excavation was approximately 15 feet deep lacked an adequate protective system, such as appropriate sloping, benching, or shield system on all unprotected sides. This condition exposed the employees to a crushing hazard by excavation wall cave-in.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Naranjo Civil Constructors @ Harmony Ave., & Zuni Street, Broomfield, CO. On or before February 1, 2012, the employer did not ensure that a ladder, ramp or other safe means of egress was provided for the employees conducting work in an excavation. The employees had been ascending and descending the sloped north side of the excavation. Additionally, the employees were attempting to set up an extension ladder on the sloped north side of the excavation at the time of inspection. This condition exposed the employees to a crushing hazard by excavation wall cave-in.
