Where the drive extends over other machine or working areas, protection against falling shall be provided.This subparagraph does not apply to manually operated sprockets. On or about 02/16/12, at the Quincey Tunnel a guard was missing on the chain and sprocket drive for the internal conveyor.

Equipment found damaged or defective shall not be used until repaired; On or about 02/16/12, at the charging station for the Tennant Floor Sweeper, the cord set was missing a ground pin.
