29 CFR 1910.219(f)(3):     Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more thanseven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided.This subparagraph does not apply to manually operated sprockets.      On or about 02/16/12, at the Quincey Tunnel a guard was missing on the chain and sprocket drive for the internal conveyor.

29 CFR 1910.304(b)(3)(ii)(C)(3):     Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired;      On or about 02/16/12, at the charging station for the Tennant Floor Sweeper, the cord set was missing a ground pin.

(a)      On or about 02/16/12, the upper meazzanine area above the entrance to the womens restroom did not have a toe board installed to prevent materials/equipment from falling to the floor level and striking employees.
