29 CFR 1910.146(c)(4):     When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:    a. A confined space program (in writing) was not developed and implemented for employees that entered tanks, conducting tasks such as welding.  A program in compliance with the standard requirements as stated in 1910.146 paragraphs (c) through (k) was not developed and written, including evaluation and documentation of possible hazards that could occur and controls to address the hazards, developing and implementing a written entry permit, providing adequate ventilation when entering a confined space, providing a stand-by person when the space is entered, providing for a rescue plan, and providing (and documenting) training.    Abatement documentation is required for this violation.
