NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a)  400 Princeton Court, Lakewood, NJ:  The scaffold being used by the employee was not planked at the working level, observed on or about 2/6/12.

29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    a)  400 Princeton Court, Lakewood, NJ:  The employee was using scaffolding that was not properly supported on adequately firm foundation.  The poles of the scaffold were resting on the ground, on blocks, and on uneven boards, observed on or about 2/6/12.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a)  400 Princeton Court, Lakewood, NJ:  An employee was using the side walls of the scaffold to access the upper level of the scaffold.  The scaffold was not designed for this function, observed on or about 2/6/12.
