OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause serious physical harm to employees in that employees were exposed to crushing hazards from falling loads due to hoist hook not being equipped with a safety latch. (a) CAMACO Lorain Manufacturing, 3400 River Industrial Park Road, Lorain, Ohio: On or about February 13, 2012, the employer did not ensure that the hook on hoist #5 at tube bender #2 was equipped with a safety latch. Hoist #5, a one ton under hung hoist, was used to repeatedly lift bundles of metal tubes weighing approximately 900 pounds per bundle. Among other methods, one feasible abatement method to correct this hazard is to ensure the hoist hooks are equipped with safety latches in accordance with the American Society of Mechanical Engineers (ASME) B30.16-2007 Overhead Hoists, section 16-1.2.9.

29 CFR 1910.176(b): The employer did not ensure that storage of material did not create a hazard: (a) CAMACO Lorain Manufacturing, 3400 River Industrial Park Road, Lorain, Ohio: On or about February 2, 2012, the employer did not ensure that pallets of material stored on the top shelf of a storage rack were secured from falling. Small boxes of manufacturing components were not adequately secured from sliding and fell approximately 14 feet to the work floor exposing employees to a struck-by hazard.
