29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. a.) Overhead beams were not frequently cleaned. On 2/10/2012, a fire occurred on Paper Machine #1 which spread to the upper area of the plant, where beams were covered with approximately 5 inches of combustible dust. Abatement Certification is required.

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location: a.) As of 2/13/2012, employees cleaned up dusty areas using an industrial shop vacuum, Troy Bilt Model #24A-204B766, to vacuum paper dust which was analyzed as a Class II dust. This equipment was not approved for a Class II location. Abatement Certification and Documentation is required.
