29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): (a) G4S Secure Solutions (USA) Inc., 5801 Brighton Blvd., Commerce City, CO: On and before 2/14/12, the employer did not assess the workplace to determine if hazards were present, or likely to be present, which necessitate the use of PPE. The employer did not determine if respiratory, foot, head, hand, eye, and/or face protection were required when working throughout the facility. Abatement note: The employer shall verify that the required workplace hazard assessment has been performed through a written certification that shall include the following: (1) Identifies the workplace evaluated; (2) The person certifying that the evaluation has been performed; (3) The date(s) that the hazard assessment was performed; and (4) Which identifies the document as a certification of hazard assessment.

29 CFR 1910.1028(e)(7)(i): The employer did not, within 15 working days after the receipt of the results of any monitoring performed under this standard, notify each employee of the results in writing either individually or by posting of results in an appropriate location that is accessible to employees: (a) G4S Secure Solutions (USA) Inc., 5801 Brighton Blvd., Commerce City, CO: On and before 2/14/12, air monitoring was conducted in the Plant 1 North gate guard shack between 1/19-20/2012 to determine employee exposure to benzene. The employer did not post the air monitoring results in an appropriate location that was accessible to all relevant employees with similar exposures.
