29 CFR 1910.178(l)(3)(i)(G):  Powered industrial truck operators did not receive initial training on fork and attachment adaptation, operation, and use limitations:    (a)  C&H Building Construction, Inc. at 13568 58th  Street NW, Williston, ND 58801:  On or about October 31, 2011, the employer did not ensure that a forklift operator was trained to safely lift a suspended load in the steel connection process.  Employees used an Xtreme XR 842 rough terrain forklift by rigging a nylon sling in a choker around a horizontal steel I-beam weighing about 2,000 pounds, and then they looped the nylon rigging around one tine of the forklift, without securing or protecting the rigging.  The operator was not trained to prevent damage to the nylon rigging from the I-beam or the tine and was not trained on the load capacity reduction caused by off-centering the load on one tine.  The load charts and plates were not adjusted to reflect the reduction in capacity.  Conditions exposed employees to struck-by hazards.    Abatement note: 1926.602(d) provides, under powered industrial truck operator training, that the requirements applicable to construction work under this paragraph are identical to those set forth at 1910.178(l) of this chapter.

Condition exposed employee to fall hazards of up to approximately 26 feet.
