Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23. a. At the workplace in the Insert Area, on or about 02/22/2012, employees traversed a stairway to access the roof without the protection of a standard railing. This railing was missing a midrail and did not extend the entire length of the stairs.

In the workplace, in the A1 Area, a Reuland Drill Press had a rotating chuck that was inadequately guarded. d.
