29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. Job site located at 915 N 43rd Street, Omaha, NE 68131-Employees were allowed to operate a forklift at the facility that had been modified by having holes drilled into the tips of the forks without having prior written approval from the manufacturer. The Nissan 5000 forklift that is used in the shop was the lift that had the holes drilled into the tips of the forks.

29 CFR 1910.242(b): Compressed air used for cleaning. Compressed air shall not be used for cleaning purposes except wherereduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment. Job site located at 915 N 43rd Ave., Omaha, NE 68131-One employee was exposed to the hazard of using plant air to clean himself and equipment off when the air pressure was over 30 psi and the nozzle being used was a make shift nozzle and did not meet the OSHA requirements of an air nozzle. The nozzle was taken out of service by the facility safety/HR manager.
