Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.    Job site located at 915 N 43rd Street, Omaha, NE 68131-Employees were allowed to operate a forklift at the facility that had been modified by having holes drilled into the tips of the forks without having prior written approval from the manufacturer.  The Nissan 5000 forklift that is used in the shop was the lift that had the holes drilled into the tips of the forks.

Job site located at 915 N 43rd Ave., Omaha, NE 68131-One employee was exposed to the hazard of using plant air to clean himself and equipment off when the air pressure was over 30 psi and the nozzle being used was a make shift nozzle and did not meet the OSHA requirements of an air nozzle. The nozzle was taken out of service by the facility safety/HR manager.
