29 CFR 1910.133(a)(1):     The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) In the production area, employee(s) were not provided with splash-proof goggles when handling corrosive chemicals, such as but not limited to Gardoclean TP 10367. Employees were exposed to chemical burns.

29 CFR 1910.215(b)(9):     The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    (a)  The distance between the adjustable left tongue guard and grinder wheel on the Black and Decker pedestal grinder was approximately 3/4 inches, exposing employees to struck-by hazards.     (b)  The distance between the adjustable right tongue guard and the grinder wheel on the Black and Decker pedestal grinder was approximately 1 1/4 inches, exposing employees to struck-by hazards.
