29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by moving parts of tank/mixer agitators and saw blades: a/ G&W/So. Plainfield NJ Site Bldgs. #111: 1/ Production Area - Mezzanine - Three vertical tanks (mixers) approx. 4 ft. in diameter and approx. 6 ft high with portable ladder access, had no grate under the covers and/or other approved means to prevent employee from falling into the tanks while dispensing product or taking batch samples. 2/ Production Area - Room #48 - Vertical Tank #57 (mixer) approx. 3 ft. in diameter and approx. 4 ft high with portable ladder access, had no grate under the cover and/or other approved means to prevent employee access to rotating agitator while dispensing product or taking batch samples. 3/ Maintenance Shop - "Journeyman" Vertical Band Saw (used to cut stock other than wood) approx. one-half inch wide blade - the unused portion of the blade approx. 8 inches exposed, located above the cutting area had no guard. 4/ Maintenance Shop - "Wells" Horizontal Metal Cutting Saw - the unused portion of the blade approx. 10 inches on the left side of the cutting area and approx. 6 inches on the right had not guard. Guards and/or other approved means were not provided to prevent accidental contact during operastion. Observed on/or about 2/14/12.
