29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  Plainview Water District, Transfer Facility - Employees were installing roof top fans for an HVAC system while working on the roof of a transfer facility.  The employer was not using a means of fall protection and fell off the roof approximately 12ft to the ground below; on or about 2/14/12.       Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
