29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. On the first floor of the school where plumbing work was perfromed, there were holes without covers in the area where the employees were working thereby exposing employees to a trip/fall hazard.
