29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length extended over its support more than 18inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end. A. Accurate Siding & Windows worksite located at 4914 Sterlingwood Lane, Perrysburg, Ohio: On or about February 15, 2012, the employer did not ensure the 30 foot scaffold platform extended a maximum of 18 inches over the pump jack support. The platform was cantilevered approximately 5.00 feet over the support on the north end and approximately 4.00 feet on the south end of the pump jack system. Accurate Siding & Windows was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451, which was contained in OSHA inspection number 311605372, citation number 1, item number 1 and was affirmed as a final order on October 7, 2008, with respect to a workplace located at Monclova, OH.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of thissection, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4)of this section. A. Acccurate Siding & Windows worksite located at 4914 Sterlingwood Lane, Perrysburg, Ohio: On or about February 15, 2012, the employer did not ensure employees were protected from falling approximately 13.00 feet to the lowest level while working on the 30.00 foot long pump jack scaffold platform located on the east side of the two story residential structure. The north and south ends of the platform were not guarded. Accurate Siding & Windows was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451, which was contained in OSHA inspection number 311605372, citation number 1, item number 2 and was affirmed as a final order on October 7, 2008, with respect to a workplace located at Monclova, OH.
