Entire Facility - On or about February 22, 2012, all required information for column "F" on the OSHA 300 Log forms was missing. The incomplete year's OSHA 300 log forms were the following; 1. One entry on colum "F" for year 2010 was missing the object/substance that directly injured or made the person ill. 2. Four entries on column "F" for year 2011 were missing the object/substance that directly injured or made person ill. 3. Two entries on column "F" for year 2012 were missing the object/substance that directly injured or made person ill.

Building # 7/Epoxy Manufacturing Process on 2nd Floor - On or about February 22, 2012, employee(s) was required to add powder material into a mixing tank while utilizing an electrical floor fan (Doerr - 606529D C96B - 115V) that had its cord/insulation loose from its metal casing assembly.
