29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: (a) Front Range Handy Man @ 1617 13th Ave., Greeley, CO 80631: On and before 2/21/12, the controlling employer did not ensure that sub-contracted employees utilized eye protection. Sub-contracted employees conducted roofing activities, in windy conditions, which exposed employees to tear off materials as well as hazards associated with pneumatic nailers. This condition exposed employees to struck-by eye hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) Front Range Handy Man @ 1617 13th Ave., Greeley, CO 80631: On and before 2/21/12, the employer did not ensure that each employee engaged in roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection. Two laborers were engaged in roof tear-off activities on a multi-family, residential home. This condition exposed employees to a fall hazard in excess of 12ft.
