29 CFR 1910.134(c)(2)(i):  Respirator users were not provided with the information contained in Appendix D to this section when the employer determined that any voluntary respirator use was permissible:  	                                                       5936 Cornhusker Hwy., Lincoln, NE 68507 (Site-Wide): The employer did not provide information within Appendix D as required by this subpart to employees using dust mask and half-face respirators voluntarily.    Abatement certification is required for this violation.  The abatement certification sheet is enclosed with these citations.

29 CFR 1910.134(c)(2)(ii):  The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored and maintained so as not to create a health hazard to the user:	                                                       5936 Cornhusker Hwy., Lincoln, NE 68507 (Site-Wide): Employees were using dust masks and half-face respirators voluntarily prior to the employer determining that employees were medically able to use a respirator  as required by this subpart.    Abatement certification is required for this violation.  The abatement certification sheet is enclosed with these citations.
