29 CFR 1926.1053(b)(1):     Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:    On or about 02/17/12 during the roofing of a commercial structure, the employer did not ensure employees extended a portable ladder three feet above the upper landing surface.

29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On 02/17/12 during roofing activities, the employer did not ensure employees used fall protection devices/systems; thus employees were exposed to a fall of 12.6 feet.      ABOVE ALL ROOFING & SIDING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER  98123, CITATION NUMBER 01, ITEM NUMBER 01 AND WAS AFFIRMED AS A FINAL ORDER ON 12/23/11, WITH RESPECT TO A WORKPLACE LOCATED AT 1036 WILBER AVE.  LAKEWOOD, OHIO 44107.
