29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:        a) Flammable storage North end of Maintenance building- On or about 26 January, 2012, Spark producing items, metal wreath holders, were stacked in the flammable locker. Employees were exposed to a fire hazard two days prior while unloading metal wreath holders into the flammable locker by hand and throwing them into a pile onto the concrete floor with the possibility of producing sparks.

Coffee can was not labeled with the identity of the liquid, identification was made by employee who put the liquid and paint brush in the can.
