29 CFR 1904.29(b)(1):     What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.      At the work site located at 100 WalMart Drive, Jackson, Ohio, the employer did not accurately record on the 2011 OSHA 300 Log of Work-Related Injuries and Illnesses, information for injured employees in column (F) that included a description of the body part injured and the object/substance that injured or made the employee ill.    a.  For C1455531, there is no description of the employee's activity at the time of the injury.    b.  For C1457456, there was no description of the employees activity at the time of the injury or what object/substance injured the employee.    c.
