At the work site located at 100 WalMart Drive, Jackson, Ohio, the employer did not accurately record on the 2011 OSHA 300 Log of Work-Related Injuries and Illnesses, information for injured employees in column (F) that included a description of the body part injured and the object/substance that injured or made the employee ill. a.
