Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.    a.  On or about March 14, 2012, in the warehouse where forklifts are used, there was a group of holes in the forklift aisles in the south bottle aisle more than one half inch deep and six inches across, causing a material handling hazard.
