29 CFR 1910.1025(l)(1)(ii):  The employer did not train each employee subject to exposure to lead at or above the action level in accordance with the requirements of this section:    On or about March 14, 2012, at the Billet Yard, an employee required to torch-cut lead billets was exposed to airborne lead at an 8-hour time-weighted average level of 160 micrograms per cubic meter of air (ug/m3), approximately 3.2 times the 8-hour time-weighted average permissible exposure limit of 50 ug/m3, during a 95 minute sampling period.  The employer did not train the employee in accordance with the requirements of this section, including, but not limited to, the contents of the standard and Appendices A and B.
