29 CFR 1910.132(d)(2):     The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:    The employer did not develop and document a personal protective equipment hazard assessment for the tasks performed by employees using the Frymaster to cook frozen food items and to perform cleaning duties near the mobile freezer and there was no written certification that included the name of the person who certified that the evaluation had been performed, and the date that the hazard assessment was completed.
