Protection shall be provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a. At the site located along Idlewild Dr in Reynoldsburg, Ohio, there was an occupied excavation without cave-in protection, that had the spoil pile placed directly against the edge of the excavation. The spoil pile was approximately three feet in height, at the edge of the excavation.

At the site located along Idlewild Dr. in Reynoldsburg, Ohio there was an employee working in an excavation, approximately six feet in depth, without any form of cave-in protection.

At the construction site located on Idlewild Dr. in Reynoldsburg, Ohio the employer had an aluminum trench box that was was constructed using Grade 8 bolts to lock the cross braces in place rather than the manufacturer's original equipment, and had not received written permission to do so.
