29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. Throughout facility - On or about February 9, 2012, the employer did not establish an energy control program for employees performing equipment repairs on a 6 color Heidelberg Speed master, and other operating equipment, thereby exposing employees to the hazard of unexpected energization of machinery.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. Throughout facility - On or about February 9, 2012, the employer did not perform an examination of the powered industrial equipment to check for defects, thereby exposing employees to struck by hazards.
