29 CFR 1926.502(d)(16)(iii):   Personal fall arrest systems were not rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level; An employee fell from through a temporary work surface was was not stopped by the fall arrest system he was using because it was not properly configured.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    On or about 02/10/2011    Employee were not trained in the proper use of fall protection systems and how to recognize and eliminate or minimize exposure to falls.
