Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes: a. In the maintenance building, the horizontal V-belt and pulleys on the Air Technologies air compressor (model: commander; S/N: L6/28/2001-00041) were not guarded on the back and bottom sides.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section: a. 29 CFR 1910.213(h)(1): The upper hood shall completely enclose the upper portion of the blade down to a point that will include the end of the saw arbor. The upper hood shall be constructed in such a manner and of such material that it will protect the operator from flying splinters, broken saw teeth, etc., and will deflect sawdust away from the operator. The sides of the lower exposed portion of the blade shall be guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed: a. In the truss shop, the blade on the Speed Cut radial saw (model: SSA-17; S/N: AB 2298) was not adequately guarded. STRAIT & LAMP GROUP, DBA FIFTH AVENUE LUMBER WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.213(h)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 98757, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 19, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 479 E. FIFTH AVENUE, COLUMBUS, OHIO 43201.
