29 CFR 1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment Mechanical Refrigeration Space: On or about February 12, 2012 and at times prior thereto, where evidence showed the degradation of piping and vessel insulation and vapor barrier, the employer had not established and implemented written procedures to conduct regular inspections of insulated piping and vessels for corrosion. This condition exposed employees to inhalation and eye and skin burn hazards to anhydrous ammonia from corrosion-induced failures of the affected piping and vessels.

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. (a) Refrigeration Space: On or about February 12 2012 and at times prior thereto, direction of flow and refrigerant state lableling was not evident on certain components of the process. For example, Piping Identification and labeling was not being maintained on Oil Separator Discharge #3, liquid ammonia pumps numbers 2, 3, and 4, and heat exchangers per the employer's Process Safety Management (PSM) Program which incorporated by reference IIAR Bulletin 114, "Identification of Ammonia Refrigeration Piping and System Components." This condition exposed employees to inhalation and eye and skin burn hazards in the event of an uncontrolled release of anhydrous ammonia. (b) Referigeration Space: On or about February 12 2012 and at times prior thereto, valve identification tags were either missing, removed, unreadable, or deteriorated to the points where they could not identify the specific valve. For example, the main discahrge valve on liquid ammonia pump #4 was not labeled per the employer's Process Safety Management (PSM) Program which incorporated by reference IIAR Bulletin 109, "Identification of Ammonia Refrigeration Piping and System Components." This condition exposed employees to inhalation and eye and skin burn hazards in the event of an uncontrolled release of anhydrous ammonia.
