A) Workplace- Stairway between main and basement levels, there was a live electrical outlet being used to plug in an adjacent conveyor belt. The outlet was broken loose from its mounting surface exposing live electrical connections; on or about 2/23/12. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
