29 CFR 1910.157(e)(3):     Portable fire extinguishers were not subjected to an annual maintenance check:    a. Where hot work was conducted, not all portable fire extinguishers were tested annually.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.178(l)(6):     The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):    a. Where a forklift was used to process scrap metal, including vehicles, the employer did not certify that the operator employee was trained (according to the osha requirements, which include "classroom" type training, demonstration of the equipment, and testing/observation of the equipment being used by the employee).    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.303(b)(2):     Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a. A few relocatable power taps (power strips) were used in the shop to power equipment such as a grinder, drill press, and other equipment.  (Note: power strips are not approved with the use of industrial type equipment and tools, but for home and computer use.  Power strips are only approved to be plugged into a building outlet.)    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
