29 CFR 1910.23(a)(9):     Every floor hole into which persons cannot accidentally walk (on account of fixed machinery,equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to preventtools or materials from falling through.    The employer did not ensure that a floor hole with an opening of more than 1 inch wide was covered in that on or about February 23, 2012 a floor hole on Plating Line #11 work platform measuring 5 1/2 inches by 5 1/2 inches was not covered creating a tripping hazard.

29 CFR 1910.133(a)(1):     The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    The employer did not ensure that employees wore face protection when transferring nitric acid (corrosive) from a 5 gallon container to a 1 gallon container and manually pouring into the plating tanks.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.      The outer insulation on the power cord for the parts hopper on Line #14 had pulled away from the bushing creating strain on the interconnectors/fittings.
