Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.    a. Facility - On or about February 22, 2012 the electrical breakers of the motor control center were not adequately marked or labeled to indicate their purpose, thereby exposing employees to the hazards associated with electrical shock.

29 CFR 1910.305(g)(1)(iii):     If used as permitted in paragraphs (g)(1)(ii)(C), (g)(1)(ii)(G), or (g)(1)(ii)(I) of this section, the flexible cord shall be equipped with an attachment plug and shall be energized from an approved receptacle outlet.    a. Off hire workstations - On or about February 22, 2012, the flexible cords used to power the electric hoists were not equipped with an attachment plug.  Employees were thereby exposed to the hazards of electrical shock.

Off hire workstations - On or about February 22, 2012, flexible cords were used as a substitute for the fixed wiring of receptacle outlets.  Employees were thereby exposed to the hazards of electrical shock.

The employee(s) were not provided with electrical protective equipment including but not limited to fire retardant clothing, hard hat, face shield, safety glasses and voltage rated gloves.  The employee(s) were thereby exposed to the hazards of electrical shock and arc flash/blasts.
