29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about February 21, 2011 and at time prior thereto employees were exposed to caught in and amputation hazards associated with unguarded rotating parts. a. Employees were exposed to caught in and amputation hazards while operating the Graziano Tortano Lathe because of the unguarded chuck. b. Employees were exposed to caught in and amputation hazards while operating the Pratt Whitney lathe because of the unguarded chuck. c. Employees were exposed to caught in and amputation hazards while operating the UR SUS 20 lathe because of the unguarded chuck. d. Employees were exposed to a caught in hazard while operating the Bridge Port Mill press #1 because of the unguarded chuck. e. Employees were exposed to a caught in hazard while operating the Bridge Port Mill press #2 because of the unguarded chuck. f. Employees were exposed to a caught in hazard while operating the Bridge Port Mill press #3 because of the unguarded chuck. g. Employees were exposed to a caught in hazard while operating the Milwaukee Mill press because of the unguarded chuck. h. Employees were exposed to a caught in hazard while operating the Clausing drill presses # 1, 2, 3, 4, 5, 6, 7, and 8 because of the unguarded chucks. i. Employees were exposed to a caught in hazard while operating the Jet drill press because of the unguarded chuck.
