You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Located at Hidaka USA, Inc. in Dublin OH: a) On the 2009 OSHA 300 log, the employer did not include all information that was required in Column F. 1. Case number 01-09 does not include the cause of the laceration to the left index finger. 2. Case number 02-09 does not include the cause of the broken right ring finger and deep laceration. 3. Case number 03-09 does not include the cause of the laceration to the front of the left leg. 4. Case number 04-09 does not include the cause of the torn tendons in the right middle finger. 5. Case number 05-09 does not include the cause of the back sprain/strain. 6. Case number 06-09 does not include the cause of the laceration and contusion to the left thumb.
