The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)    JOB SITE: 6827 Vernon Avenue, St. Louis, MO 63130, at this work site, a 8 foot step ladder was used to gain access onto the roof, approximately 10 feet above next lower level, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface.  The workers were exposed to a fall hazard.

The worker were exposed to a fall hazard.

The program did not enable each employee to recognize hazards related to ladders and stairways, and did not train each employee in the procedures to be followed to minimize these hazards:    JOB SITE: 6827 Vernon Avenue, St. Louis, MO 63130, the employer failed to provide a training program on the use of step ladders; the employer allowed his workers to use the step ladder to step on and off the roof.  The workers were exposed to a fall hazard.
