29 CFR 1915.89(e)(3): The employer did not use an orderly shutdown to prevent exposing any employee to risks associated with hazardous energy: Factory Deck, Double Fish Header: On or about 02/14/2012 and at times prior thereto, the equipment was re-energized prior to being completely restored to operational condition. A clear and orderly manner of re-assembling and returning to service was not initiated by the employer. This condition exposed employees to amputation hazards in the event the equipment unexpectedly started up.
