29 CFR 1915.89(c)(2):  The employer did not ensure the use of a lock to prevent energization or startup, or the release of hazardous energy, before any servicing was started:    Factory Deck:  On or about February 15, 2012 and at times prior thereto, a lock was not used to ensure the Model Number PHC550R, Machine Number 123, Tokai Header to prevent energization or start-up.  This condition exposed employees to amputation hazards when replacing the blade.

29 CFR 1918.103(a):  The employer did not ensure that each affected employee wore a protective helmet when working in areas where there was a potential for injury to the head from falling objects:    Coastal Dock:  On or about February 15, 2012 and at times prior thereto, an employee was observed not wearing a protective helment during an active longshoring operation.  This condition exposed the employee to head impact hazards from poorly secured bags of frozen fish weighing up to 44 pounds.

29 CFR 1910.1200(e)(1)(i):  The written hazard communication program did not include a list of the hazardous chemicals known to be present, using an identity that was referenced on the appropriate material safety data sheet:    Factory Deck:  On or about February 15, 2012 and at times prior thereto, the inventory of hazardous chemicals was not inclusive of all hazardous chemicals at the work site.  This condition hampered the efforts of the employees to find accurate chemical information.

29 CFR 1910.1200(g)(8):  The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace:    Factory Deck:  On or about February 15, 2012 and at times prior thereto, a copy of the material safety data sheet for the factory cleaner "Harbor Master" was not maintained by the employer.  This condition hampered the employee's ability to obtain specific chemical information and exposed them to skin, eye, ingestion, and inhalation hazards.
