29 CFR 1915.56(b)(2): Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected:  F/V Frontier Mariner, engineering area: on or about 06 February 2019, employees welding with a cable of approximately 10 feet had electrical tape over the wiring were exposed to electrical shock and burn hazards.

29 CFR 1918.62(a)(2): Defective gear, as defined by the manufacturer's specifications, was used by the employer.        a)  F/V Frontier Mariner, weather deck, bow freezer hold hatch wire sling: on or about 06 February 2019,  two "mouse and hooks" attached to the freezer hold hatch wire sling were inoperable and not secured correctly to prevent disengagement exposed employees to struck-by and crushing hazards from falling loads.      b)  F/V Frontier Mariner, weather deck, frozen fish offload operations: on or about 06 February 2019, a wire sling used to offload pallets of boxed frozen fish did not have a safety wire installed in the "scoop and shackle" exposed employees to struck-by and crushing hazards from falling loads.

29 CFR 1918.62(b)(1): The employer did not ascertain and adhere to the manufacturer's recommended ratings for wire rope and wire rope slings, and/or did not have such rating available for inspection.    F/V Frontier Mariner, vessel bow, freezer hold area: on or about 06 February 2019, wire rigging without load indicator markings was used to offload boxed frozen fish product from the vessel's freezer hold weighing an average of approximately 2,400 pounds per pallet exposed employees to struck-by and crushing hazards from falling loads.
