29 CFR 1910.147(c)(4)(ii)(D):     The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control:    a. Not all specific operating procedures stated a positive, active method for testing equipment, such as, but not limited to, for the Centerless Grinder, where the test for isolation of electric energy was to observe if the wheel stopped spinning, for CNC's, where the test for isolation of electric energy was to observe panel lights, for the Assembly Machines (D1100001), where the test for isolation of electric energy was to "Try To Start," however, the SOP did not state how to try to start.  (Note: SOP's should be reviewed for all test methods for positive, active test methods.  "Observation of Ball Valves," for example would not be a specific procedure where a control button could be engaged to ensure an air or fluid line is locked out.  Likewise, observation of pressure gauges would not be a specific test, if a more positive, active test could be used; unless an observation is the only method available.)    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.147(c)(7)(iv):     The certification of employee training by the employer did not contain each employee name and date of training:    a. Lockout training records were not kept for each employee, whether authorized or affected (training records were not available for every employee).    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
