29 CFR 1926.56(a):  Construction areas, ramps, runways, corridors, offices, shops, and storage areas were not lighted to at least the minimum illumination intensities listed in Table D-3 while any work was in progress:      a)  On or about February 21, 2012, and at times prior, employees were exposed to trips and falls because the employer had not provided sufficient lighting in areas through which employees had to walk and carry equipment to containment areas on the second floor and in the basement.

29 CFR 1926.405(g)(2)(iv):  Flexible cords were not connected to devices and fittings so that strain relief was provided which would prevent pull from being directly transmitted to joints or terminal screws:    a)  On or about February 21, 2012, and at times prior, employees were exposed to electrocution and shock hazards because an extension cord used by employees near a decontamination unit was not provided with adequate strain relief which allowed for the outer and inner insulation to be stressed and damaged, thereby exposing the inner conductive material.

29 CFR 1926.1101(g)(3)(iii):  Dry sweeping, shoveling or other dry clean-up of dust and debris containing ACM and PACM shall not be used:    a)  On or about February 21, 2012, employees were exposed to asbestos because the employer allowed them to dry sweep debris and dust after part of a wall in the office area which was coated with asbestos containing materials was demolished.
