29 CFR 1910.28(a)(4): Scaffolds and their components shall be capable of supporting without failure at least four times the maximum intended load. A.) On or about February 6, 2012 at 67-29 Metropolitan Avenue, Middle Village, NY 11379 While working/standing on a wooden plank following the lowering of a steel vault into a cemetery plot three (3) employees fell approximately 65 inches (approximately 5 1/2 feet) to the lower level as a result of the work platform's failure to support at least four times the maximum intended load.
