29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Industrial Farm Tank, Inc. - Lewistown, Ohio: On or about March 7, 2012, the employer did not develop and implement an energy control program. Employees were exposed to injuries when performing tasks such as, but limited to, changing bearings on The McNeil Machine Rotocast oven. The oven was not locked out during the maintenance activity. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts: a) Industrial Farm Tank, Inc. - Lewistown, Ohio: On or about March 15, 2012, employees operating the McNeil Machine Rotocast Oven were exposed to struck-by injuries in that the rotating arms were not guarded. Employees were not protected during a time in which they accessed the hazard area of the machine while another employee controlled the machine. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
