29 CFR 1926.652(a):     Each employee  in an excavation was not protected from cave-ins by an adequate protective system.    At the site, in an excavation with 12' to 16' deep walls, employees were installing the plywood framing for a pour-in-place junction box.  The absence of protective systems or benching exposed workers to possible cave-in hazards.
