29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and projectiles. 1. Employees were exposed to struck by hazards when the metal shredder was operated. The metal shredder was ejecting shrapnel when it was operated.

29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year: The OSHA 300A forms had deficiencies as follows: 1.) The Number of Cases, fields G-J, was not populated for the one injury in calendar year 2009. 2.) The Number of Cases, fields G-J, was not populated for the two injuries in calendar year 2011.
