29 CFR 1910.134(c)(1):     The employer did not ensure that a written respiratory protection program with worksite-specific procedures was established and implemented to include the provisions of (i)-(ix), as applicable    On or about February 24, 2012, and at times prior thereto, the employer had not developed and implemented a written respiratory protection program for employees using respirators to protect against MDI.  Employees were exposed to 0.31 mg/m^3 of MDI while spraying bedliners.  Elements of a required respiratory program include, but are not limited to, medical evlauations for employees required to use respirators, fit testing for tight fitting respirators, training in use and care of respirator, and a change-out schedule for respirator cartridges.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a respiratory protection program is developed, implemented, and maintained for all employees required to be in the spray booth.

29 CFR 1910.305(b)(1)(ii):     Unused openings in cabinets, boxes, and fittings were not effectively closed.    On or about February 24, 2012, and at times prior there to, a 120 volt receptacle box (metal junction box) was observed with one pre-punched knock-out holes in an open condition.  The knock-out had been punched out, leaving the metal box open to dust, debris and penetration by conductive objects (fingers, screwdriver, other tools), exposing employees to the hazards of electrical shock.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the knock-out is plugged or the box replaced.

29 CFR 1910.305(b)(2):     Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    On or about February 24, 2012, and at times prior thereto, a metal junction box was observed with the cover missing, leaving the metal box open to dust, debris and penetration by conductive objects (fingers, screwdriver, other tools, etc.), exposing employees to the hazard of electrical shock.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the cover on the metal junction box is replaced.

29 CFR 1910.305(g)(1)(iv)(A):     Flexible cords and cables were used as a substitute for the fixed wiring of a structure:    On or about February 24, 2012, and at times prior thereto, an extension cord was being used as permanent wiring for the air compressor drier.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the drier is connected to permanent wiring.
